• Portuguese-American 

• Attending college, allied health, or professional school in New England

• ESSAY (up to 500 words) addressed to Dr. Edward Leitão Memorial Scholarship Fund Committee, describing your language and cultural connections to the Portuguese-American community, your goals,   and how you envision making a difference once you finish your degree

•  Two letters of recommendation (from  teachers/deans/academic advisors, etc) addressed to Dr. Edward Leitão Memorial Scholarship Fund Committee

• copy of transcripts 

• copy of SAT or ACT scores, if applicable
