Eligibility:

• Portuguese or Portuguese-American 

• MD degree obtained outside of the US

• In the process of  applying for residency in the US

• ESSAY (up to 500 words) addressed to Dr. Edward Leitão Memorial Scholarship Fund Committee, describing your language and cultural connections to the Portuguese-American community, your goals, and how you envision making a difference once you finish your Residency Program 

•  Two letters of recommendation (from  teachers/deans/academic advisors/rotation directors, etc.) addressed to Dr. Edward Leitão Memorial Scholarship Fund Committee

• copy of Medical School transcripts 

• copy of MD Degree
